Grades 6-8

School Starts August 6th. Full Time Enrollment Ends September 11th.

If you are seeking full-time enrollment with Utah Online, please submit the following documents:

  1. TWO Proofs of Residency. Examples include:
    • Drivers License
    • Mortgage Statement
    • Rental Contract
    • Recent Utility Bill (Within 60 Days)
  2. Birth Certificate.
  3. Immunizations or Health Department Exemption.
  4. Utah Online Exemption Form (Please click to download.)
    • All students must submit this form, regardless of where they reside in Utah.
    • Please indicate Utah Online as the school.
    • Utah Online will complete the district signature at the bottom of the form.  
  5. Safe Schools Conduct Statement (Please click to download.)
  6. Special Education Papers if applicable (IEP and IEP Eligibility Form, 504 Plan.)

To officially enroll in our school, you must submit ALL required documents.

Grades 9-12

To be eligible for full-time enrollment, students must be no more than 3 credits behind in their graduation requirements.  If students do not meet this requirement, part time enrollment is available. 

If you are seeking full-time enrollment with Utah Online, please submit the following documents:

  1. TWO Proofs of Residency. Examples include:
    • Drivers License
    • Mortgage Statement
    • Rental Contract
    • Recent Utility Bill (Within 60 Days)
  2. Birth Certificate.
  3. Immunizations or Health Department Exemption.
  4. Transcript (if applicable – only for grades 9-12 or if you have previous high school credit.)
  5. Utah Online Exemption Form (Please click to download.)
    • All students must submit this form, regardless of where they reside in Utah.
    • Please indicate Utah Online as the school.
    • Utah Online will complete the district signature at the bottom of the form.

The Online Exemption Form ensures that parents and students understand that all graduation requirements must be met in order for students to earn a high school diploma with Utah Online.
